DEVICE TEST REPORT FORMS
For our customers we have posted a new test report form that has several places for information pertaining to the newer devices that are out.
This form allows for the identifying of the new special fire line devices and the spill proof vacuum breaker.
Some water suppliers supply their own forms that they require be used. If you have to supply your own forms, this might work for you. The form has room at the top to insert your logo or company information.
We have posted two formats, one in PDF format and one in Microsoft word. Hopefully you will be able to use these forms to have your own three part NCR forms made.
If you are unable to insert your information or do not have easy access to a print shop, we can produce a camera read copy with your information for $20.00. We can provide three part NCR forms with your information for $48.00 per 100 + the one time set up charge of $15.00.
